                      CASE HISTORY

PATIENT DETAILS:

A 75 years old female patient presented with

CHIEF COMPLAINT:

Swelling of lower limbs and face puffiness  since 9 months

HISTORY OF PRESENT ILLNESS:

Patient was apparently asymptomatic 9months ago then she developed edema in lower limbs bilateral and puffiness of face. She visited local hospital and gone through various routine tests and diagnosed as kidney failure and undergone dialysis from past 8 months symptoms has been subsided but these are reccuring frequently and progressed gradually upto present stage. 

HISTORY OF PAST ILLNESS:

Diabetes mellitus: present since past 15 years

Hypertension: present since past 2 years

PERSONAL HISTORY:

Diet : mixed diet

Appetite: Decreased

Bowel and bladder: normal

Addictions: none

DRUG HISTORY:

GENERAL EXAMINATION:

Patient is consciousand moderate in-built

Pallor : no

Icterus : no

Cyanosis: no

Clubbing: no

Lymhadenopathy: no

Edema: no

PROVISIONAL DIAGNOSIS:

Chronic kidney failure

QUESTIONS:

How frequently should i come for dialysis?
